Grace Lutheran Church Education Permissions
Media Clause: I hereby grant Grace Lutheran Church permissions to record any child(ren)'s likeness and/or voice for use in television, radio, printed material, social or printed media to further the aims of the Sunday School program of Grace Lutheran Church in areas such as : radio, cable, newsletter, bulletin, newspaper, website, or Facebook.

Emergency Clause: In the event I cannot be reached in an emergency, I hereby give my permission to the employees of Grace Lutheran Church and Sunday School Coordinator to secure medical care for my child as deemed necessary.

Release Clause: The undersigned hereby release and hold harmless Grace Lutheran Church its employees or agents thereof, including without limitation the Sunday School Program from any and all claims, liabilities, or demands whatsoever arising or claimed to have arisen out for the enrollment of participation in any program by the participant herein.
